At Purpose Driven Living LLC, we understand that choosing a residential setting is an important decision for the individual, their family, guardian, and support team.
Purpose Driven Living LLC is currently preparing for future AFC operations and is not yet accepting resident placements. We welcome inquiries from families, guardians, case managers, support coordinators, and community partners interested in learning more about our developing program.
Our future admissions process is designed to be thoughtful and person-centered. Each potential placement will be considered individually to determine whether the person's assessed needs, preferences, required level of care and supervision, and support needs can be safely and appropriately met within our program.

Finding the Right Fit
A diagnosis or funding source alone does not determine admission. Potential placements will be evaluated individually and may include consideration of:
- Assessed care and supervision needs
- Personal preferences and goals
- Health and safety needs
- Behavioral and support needs
- Mobility and accessibility needs
- Medication-related support needs
- Compatibility with the home's program and environment
- Available staffing and resources
- Compatibility with other residents
- Funding and authorization requirements, when applicable
- Whether Purpose Driven Living can safely and appropriately meet the individual's needs
Information That May Be Requested
When Purpose Driven Living begins accepting referrals, documentation requested during the referral and admission process may vary based on the individual and referral source.
Information may include assessments, individual plans, medical or medication information, behavioral or support information, guardianship or authorized representative documentation, emergency contact information, funding/authorization information, and other records necessary to evaluate and safely plan a potential placement.
Please do not submit medical records, Social Security numbers, detailed health information, or other sensitive personal information through the general website contact form. We will provide instructions for securely submitting required documentation when appropriate.

Purpose Driven Living welcomes opportunities to connect with case managers, support coordinators, Community Mental Health organizations, social workers, discharge planners, and other community partners. While we complete the steps required for AFC operations, referral professionals are welcome to contact us to learn more about our developing program, intended population, approach to residential support, and future referral opportunities.
Our Future Referral Process
When Purpose Driven Living is licensed and accepting referrals, placements will be considered through the following process.
Initial Inquiry
A family member, guardian, case manager, support coordinator, individual, or other authorized referral source may contact Purpose Driven Living to learn about the home and discuss potential future placement needs.
Information & Assessment Review
When Purpose Driven Living is licensed and accepting referrals, appropriate information regarding the individual's needs, preferences, abilities, required supervision, supports, and individual plan may be reviewed to help determine whether our program could be an appropriate match.
Compatibility & Program Review
We will consider whether the individual's needs can be safely and appropriately supported within the home's approved program, staffing capabilities, available resources, resident composition, and level of care and supervision.
Admission Planning
When a placement is determined to be appropriate and all applicable requirements are satisfied, Purpose Driven Living will coordinate with the individual and authorized members of the support team regarding the admission process, required documentation, transition planning, and next steps.

Purpose Driven Living LLC is currently completing the process required to operate as a Michigan Adult Foster Care home. We are not yet licensed and are not currently accepting resident placements or providing licensed AFC services. We welcome general inquiries and opportunities to connect with families, guardians, referral professionals, and community organizations as we prepare for future operations.
